A progress bar tracks steps while a carousel advances through satisfaction radios, aspect ratings, and guided prompts introduced with alerts. Navigation and submit controls move shoppers through the post-purchase questionnaire.
Structured step chrome with progress as the primary orientation cue. Alerts frame context at the start of steps. Radio groups keep answers fast and consistent across aspects.
A moderate to elaborate feedback flow compared with single-screen forms. Content-driven by question copy and rating scales. Distinctive for carousel steps plus progress inside a sheet.
Steps remain one question group at a time on mobile, which keeps the sheet usable despite longer surveys.
